6-Question Form 8 X 11 Laminated Board

This laminated board has 2 sides; the 6 Question Form is on one side and a four-directional body chart on the other. 

This board coordinates with the Massage Your Market training which teaches you about the 6 questions you use to design the massage flow to meet client expectations. This laminated board makes it easy to streamline your client notes from this erasable board to an online charting system or SOAP notes.

The other side of the board is a body chart that allows your client to "x" the areas they would like focused massage work for. This board is also used in the PPS bodywork mapping training.
